Los Papilomavirus (VPH) son más de 120 tipos, de los cúales hay un grupo que puede infectar el tracto genital inferior.
Suelen ser asintomáticos, a veces pueden dar lugar a prúrito y dolor. Algunos inducen verrugas genitales o condilomas acuminados, de localización variable, en cuello uterino, vagina, vulva, ano, pene, escroto, periné o ingles.
La primoinfección suele producirse antes de los 30 años y afecta por igual a ambos sexos.
¿Qué medios hay para evitar la infección por VPH?
Dado que las infecciones genitales por VPH se transmiten por vía sexual es fundamental la adopción de medidas profilácticas para evitar el riesgo de contagio.
Están aprobadas por la FDA vacunas muy efectivas, una tetravalente y otra bivalente para los tipos 16 y 18 (provocan la mayoría de los cánceres cervicales) y los tipos 6 y 11 que dan lugar a las verrugas genitales.
¿Qué puede producir una infección por estos virus?
Algunos papilomavirus de riesgo oncogénico (entre 20 y 25 tipos de virus) son la causa principal del cáncer de cérvix que afecta a más de medio millón de mujeres cada año en todo el mundo. También se ha demostrado su participación en las neoplasias de vulva, vagina, ano, pene y neoplasias orales.
Existen además algunos factores favorecedores del cáncer cervical: el tabaco, las hormonas, la inmunodepresión e infecciones sexuales de repetición.
¿Qué medios hay actualmente para el diagnóstico de estas infecciones?
Dado que la mayoría de las infecciones por VPH no producen síntomas exceptuando las lesiones por condilomas acuminados, disponemos del frotis cervical (citología-Papanicolau), colposcopia y test específicos para la determinación de VPH.
¿Cúales son los tratamientos que disponemos hoy en día?
Las infecciones por Papilomavirus son muy frecuentes, aunque afortunadamente pocas veces producen alteraciones neoplásicas, permaneciendo en su mayoría latentes sin volver a reactivarse. La presencia de una lesión precancerígena cervical (CIN) producida por el VPH se trata mediante una técnica quirúrgica denominada conización, siendo la modalidad con asa-láser la más recomendada y exitosa.
Las verrugas genitales (condilomas acuminados) se tratan con aplicaciones tópicas de Iniquimod o Podofilina o bien mediante electrocauterización, crioterapia o tratamiento con láser; éste último con la ventaja de su menor grado de recidiva y rapidez en la desaparición de las lesiones.
La terapia con láser permite una vaporización selectiva del condiloma o verruga, facilitando la cicatrización de la zona tratada y minimizando las molestias post-tratamiento, ya que provoca una menor inflamación y edema.
La aplicación del láser ofrece una alta precisión en superficie y profundidad de las lesiones, por lo que la tasa de éxito es alta, llegando a un 89% y con recidivas del 8% durante los siguientes 2 años, frente al 25-30% de las alternativas convencionales.

Papillomavirus (HPV) is a family of viruses with more than 120 types; among them there is a group that can infect the inferior genital track.
They tend to be asymptomatic, and at times can lead to itching and pain. Some cause genital warts or condylomata acuminata, in various locations, in the cervix, vagina, vulva, anus, penis, scrotum, perineum, or groin.
The primary infection tends to appear before 30 years of age and affects both sexes.
What are the ways to avoid the HPV infection?
Given that genital HPV infections are transmitted through sexual contact it is fundamental to take prophylactic measures in order to avoid the risk of infection.
There are very effective vaccines that are approved by the FDA, one is tetravalent and the other bivalent for types 16 and 18 (which cause the majority of cervical cancers) and types 6 and 11 that can cause genital warts.
What can an infection produced by this virus cause?
Some papillomaviruses with oncogenic risk (between 20 and 25 types of the virus) are the main causes of cervical cancer that affect more than half a million women each year all around the world. Their participation has also been demonstrated in neoplasia of the vulva, vagina, anus, penis, and oral neoplasia.
There are also some predisposing factors related to cervical cancer: tobacco, hormones, immunosuppression and repetitive sexual infections.
What current methods are there for the diagnosis of these infections?
Given that the majority of these HPV infections do not produce symptoms, except for lesions caused by condylomata acuminata, we use the cervical smear (Pap smear), colposcopy, and other specific tests in order to determine the presence of HPV.
What are the current available treatments?
The infections caused by the Papillomavirus are very frequent, although fortunately very few times they produce neoplasia alterations, the majority remaining latent without reactivation. The presence of a precancerous cervical lesion (CIN) produced by HPV is treated through a surgical technique called conization, which is the form with asa-laser and is the most recommended and successful method.
Genital warts (condylomata acuminata) are treated with topical applications of Imiquimod or Podofilina or through electrocautery, cryotherapy, or laser treatment; the latter with the advantage of its lower rate of recurrence and rapid disappearance of the lesions.
Laser therapy allows for selective vaporization of condylomata or warts, facilitating the healing of the treated area and minimizing the post-treatment discomfort, as it causes less inflammation and edema.
The application of the laser offers high accuracy on the surface and depth of the lesion; therefore there is a high success rate, reaching 89% with 8% recurrence rate during the following 2 years, compared to 25-30% from conventional alternatives.
