I'm not sure if 状況 here means the situation/conditions in which symptoms occured, or the onset and development of symptoms (glova gives 'timeline'), or if it is just the overall general state of affairs regarding presentation of symptoms.

circumstances of manifestation

I would keep the literal and translate 状況 because the evaluation is a look into the particular conditions relating to the etiology (source of) the disease not into the conditions themselves. It's a fine nuance that I think would be good to keep. ... circumstances of onset / appearance would work as well.

agree Drozjp : http://www.mhlw.go.jp/topics/2006/10/dl/tp1020-2.pdf この内容の調査であれば"circumstances"は必要やと思います。この調査で一番知りたいのは、どの薬を飲んだ時に(ハッキリ言ってT)、いつ頃からどういう症状(異常行動)が出たのか、ということです。PDFの前半に出てくる質問票を見る限り「状況」も訳出する方がいいと思います。

appearance, onset

状況, as used this waym hardly add meaning and can almost be left out in English translation. 発現状況 simply means the appearance (or onset) of these symptoms. 状況 is there becasue this phrase is referring to when and how (for instance, what kind of people developed the symptoms and how the symptoms progressed) of these symptoms. (I guess now I am saying 状況 means something...)

There may be a more target-specific medical word that I don't come up with...
