This is from a survey on flu-related symptoms, entitled "インフルエンザに伴う随伴症状の発現状況に関する調査研究". Also in the text: ...発熱日から7日にわたる臨床症状(肺炎、けいれんなど)の発現状況を調査し、...
I'm not sure if 状況 here means the situation/conditions in which symptoms occured, or the onset and development of symptoms (glova gives 'timeline'), or if it is just the overall general state of affairs regarding presentation of symptoms.

circumstances of manifestation
I would keep the literal and translate 状況 because the evaluation is a look into the particular conditions relating to the etiology (source of) the disease not into the conditions themselves. It's a fine nuance that I think would be good to keep. ... circumstances of onset / appearance would work as well.

appearance, onset
状況, as used this waym hardly add meaning and can almost be left out in English translation. 発現状況 simply means the appearance (or onset) of these symptoms. 状況 is there becasue this phrase is referring to when and how (for instance, what kind of people developed the symptoms and how the symptoms progressed) of these symptoms. (I guess now I am saying 状況 means something...)

occurrence of clinical symptoms
Flu is caused by different viruses, and thus the symptoms are different.
The identification of virus is commonly made by investigating the clinical symtoms.
