On a medication guideline:

Not to be administered ***12 hours before/after scales***.

What is scale in this context? A tool to weigh? A stage in a clinical trial?

A stage in a clinical trial (most likely)

Because an administered substance wouldn't affect weight that much in 12 hrs interval (unless it is a laxative). However, substances are likely to affect substantially other parameters.

A small, thin plate of horny epithelium, resembling a fish scale, cast off from the skin

Not sure whether this suits your context at all, but scales can also refer to flakes of skin that desquamate after various dermatological conditions. It can refer to dandruff (seborrheic dermatitis) or fungal skin and nail infections.
However, knowing when they would occur in terms of time would require you to know the underlying condition - if it is not in relation to the skin, then probably not the correct meaning in this context.
