or when one is intimidated, embarrassed. Another Option would be to translate the term as to speak freely, not to beat about the bush, not trying to hide something, but to be outspoken and frank.

one of this verb´s meanings is to speak loud and clear (i.t. to enunciate)
and I think this is meant in the present context. to speak with a firm loud voice so that one´s words are intelligible (which is usually not the case if one is in a depressed mood.
