I somehow dont seem to like terms like query screen, inquiry screen. Someone suggested scanning mask which sounds pretty clumsy. Whats the best word for this?? Thanks in advance.

: A mask is only *one* part of the GUI! In this case it is a dialog box or mask (not a template).
