The preclinical safety profile of fingolimod was assessed in mice, rats, dogs and monkeys. The major target organs were the lymphoid system (lymphopenia and lymphoid atrophy), lungs (increased weight, smooth muscle hypertrophy at the bronchio-alveolar junction), and heart (negative chronotropic effect, increase in blood pressure, perivascular changes and myocardial degeneration) in several species; blood vessels (vasculopathy) in rats only at doses of 0.15 mg/kg and higher in a 2-year study, representing an approximate 4-fold margin based on the human systemic exposure (AUC) at a daily dose of 0.5 mg.
No evidence of carcinogenicity was observed in a 2-year bioassay in rats at oral doses of fingolimod up to the maximally tolerated dose of 2.5 mg/kg, representing an approximate 50-fold margin based on human systemic exposure (AUC) at the 0.5 mg dose. However, in a 2-year mouse study, an increased incidence of malignant lymphoma was seen at doses of 0.25 mg/kg and higher, representing an approximate 6-fold margin based on the human systemic exposure (AUC) at a daily dose of 0.5 mg.
Fingolimod was neither mutagenic nor clastogenic in animal studies.
Fingolimod had no effect on sperm count/motility or on fertility in male and female rats up to the highest dose tested (10 mg/kg), representing an approximate 150-fold margin based on human systemic exposure (AUC) at a daily dose of 0.5 mg.

Доклінічний профіль безпеки фінголімоду оцінювали у мишей, щурів, собак і мавп. Основними органами-мішенями були лімфоїдна система (лімфопенія та лімфоїдна атрофія), легені (збільшення ваги, гіпертрофія гладких м’язів на бронхо-альвеолярному з’єднанні) та серце (негативний хронотропний ефект, підвищення артеріального тиску, периваскулярні зміни та дегенерація міокарда) у кількох видів; кровоносні судини (васкулопатія) у щурів лише після застосування доз 0,15 мг/кг і вище у 2-річному дослідженні, що приблизно у 4 рази перевищує граничні значення, виходячи з даних системного впливу на людину (AUC) у добовій дозі 0,5 мг.
Під час 2-річного біологічного аналізу на щурах при пероральному застосуванні фінголімоду до максимально переносимої дози 2,5 мг/кг, що приблизно у 50 разів перевищує граничні значення, виходячи з системного впливу на людину (AUC) дози 0,5 мг, не спостерігалося жодних ознак канцерогенності. Однак у 2-річному дослідженні на мишах було виявлено збільшення частоти розвитку злоякісних лімфом для доз 0,25 мг/кг і вище, що приблизно у 6 разів перевищує граничні значення, виходячи з системного впливу на людину (AUC) добової дози 0,5 мг.
У дослідженнях на тваринах фінголімод не виявляв ні мутагенного, ні кластогенного впливу.
Фінголімод не впливав на кількість/рухливість сперматозоїдів або на фертильність у самців і самиць щурів до найвищої випробуваної дози (10 мг/кг), що приблизно у 150 разів перевищує граничні значення, виходячи з системного впливу на людину (AUC) добової дози 0,5 мг.

Source text - Ukrainian 2. Методика проведення випробування. Облік результатів. Випробування проводять у двох паралельних пробах. Випробовуваний розчин препарату і реактиви вносять у пробірки, як наведено у таблиці 1. Всі пробірки обережно перемішують і одночасно поміщають на 60±2 хвилини у водяну лазню або термоблок при температурі 37±1°С. Слід уникати вібрації й ударів під час інкубації та при вийманні пробірок.
Після закінчення зазначеного часу результати реєструють тільки як позитивні або негативні. Позитивним результатом вважають утворення стійкого гелю, який не руйнується при обережному перевертанні пробірки на 180°. Негативним результатом вважають відсутність стійкого гелю.
Тест дійсний, якщо в обох пробірках з негативним контролем отримані негативні результати і у всіх 4-х пробірках з позитивними контролями отримані позитивні результати.
Препарат відповідає вимогам тесту, якщо в кожній з двох паралельних випробовуваних проб отримані негативні результати. Якщо в одній із випробовуваних проб отриманий позитивний результат, а в іншій - негативний, випробування проводять, як вказано вище, починаючи з приготування випробовуваного розчину.
Якщо в кожній із двох рівнобіжних випробуваних проб отримано позитивний результат, випробування повторюють в максимально допустимому розведенні випробовуваного зразка.
Translation - English 2. Test procedure. Registration of results. The test is performed in duplicate. Add the product test solution and the reagents to the test tubes as shown in table 1. Carefully mix all the test tube contents and simultaneously place them for 60±2 minutes into a water bath or heat block with the temperature of 37 ±1oC. Vibrations and strokes should be avoided during incubation and removal of test tubes.
After expiration of the specified time interval, the results are registered only as positive or negative. The positive result is interpreted as formation of a stable gel, not damaged upon careful inversion of the tube by 180 o. The negative result is the absence of a stable gel.
The test is valid provided the negative results are obtained in both of the negative control test tubes, and positive results are obtained in all 4 of the positive control test tubes.
The product meets the test requirements provided the negative results are obtained in each of the two parallel test samples. If the positive result is obtained in one of the test samples and the negative result is obtained in the other one, the testing is performed as specified above, starting from preparation of the test solution.
If the positive result is obtained in each of two concomitant test samples, the testing is performed in maximum allowable dilution of the test sample.
