Rates for typesetting

Dear colleagues,

How much do you charge for typesetting? I assume it is an hourly rate.

All comments will be much appreciated.

If I were interested in the rates for typesetting I would ask some typesetting companies about their rates.

I was once asked to quote for both translation and typesetting, the job went to somebody else then but I was at a loss a bit... What do you think is a fair price?
