流入部

拡張した小腸を追いかけると左内鼠径輪の近傍で肥厚した腹膜が腹腔内に突出し、同部に小腸がはまり込み拡張した口側小腸が流入部直前のところで穿孔していた.

This is describing an inguinal hernia. I am not sure which part of the intestinal tract 流入部 is referring to or how best to translate it.

I figured out that this refers to the point where the bowel enters the hernial sac.

neck of the hernia, hernial neck

I believe they are referring to the neck of the hernia, which is the narrowest anatomical part, proximal or rostral to which (as indicated in your sample) the bowel will be dilated due to the accumulation of intestinal contents that cannot be released in the absence of surgical intervention. It could probably also be translated as an inflow tract, but from a surgical perspective, neck should be the correct translation.
