I am having a problem understanding the sentence structure, above all the "[...] ali da li smo mi pacijenti neugledni." part. Why is there a full stop rather than a question mark? And what's the meaning of 'neugledan' here?

Mislim da bi u tom kontekstu moglo „insignificant“ (pacijenti), za razliku od „significant“ (ugledan/značajan profesor – significant professor), mislim da se to želelo potencirati, tj. ironično naglasiti.

yes, given on retirement because he "earned" it; in other words because he "merits it" (or at least is supposed to). One way or another, being "of no merit" is still another good way of saying that this patient is of no importance.
