How are you?
Kicking, but not high.

let me try to rephrase it for you in simple English:
good, but not fantastic (great)
fine, but I¥'ve had better days

My interpretation of "kicking, but not high" is "good, but not that great" (correct me if I'm wrong). So, my answer is more of a translation of that interpretation, since direct translation would not make any sense.
Hope this helps!

The "kicking" comes from "alive and kicking," which I associate with Jimmy Durante over 40 years ago, so Mike's 死語 is most apt.
