The same abbreviated dating is used in Japanese medical records as the U.S (month/day), if this is a clinical study/trial the term "subject" should be used instead of patient.

agree Hiroaki Masuda : I agree. However, I don't think the patient went to bed at 8 pm. So, "after 8 pm while he was in bed" or something similar would be more appropriate.

Thank you and I agree with your point. I thought the wording sounded more natural and made the assumption based on the time 20:00 and wording (and assuming the subject was in the clinical trial facility).

after 8 pm on July 16, I had defecation desire while asleep

7/16 = July 16
20時過ぎに = after 8 pm
便意 = defecation desire ("call of nature" can be used but "call of nature" can mean "urination desire" as well)
