In case of Philadelphia, 12 people are listed as Mayor's cabinet members (see http://www.phila.gov/mayor/cabinet.html), and in case of Hiratsuka, 3 immediate assistants are listed (助役 and 収入役) in addition to 21 departments under Mayor's Office or 市長部局 (http://www.city.hiratsuka.kanagawa.jp/cityhall/sosiki.htm). In other words, the phrase covers quite a bit of job titles.
