When filling a form, does the above mean that you should only fill in the ZIP code of the address?

説明:
I think, if there are other fields provided in the form for mentioning the address, then this field asks only for the 'ZIP code'. If not, entering both (address & ZIP) is expected.
Also, from the size of the field, you can make out if it asks to enter only a ZIP or a full address.
Then depending upon suitability, you can say just 'ZIP' or 'ZIP code' or 'address/location with ZIP'.

賛成  Hidetoshi kudo: Agree with your "address & ZIP". However "ZIP" sounds too American. Does any one think "post code" is suitable for Japanese?

I believe the 〒(zip code mark) has been inserted to ensure that the zip code is also written down in the section, as a lot of people forget to do so when filling a form.
The reason it is written next to 所在地 is simply because in Japan 〒, or ZIP code, comes in front of the rest of the address.
